For the cross-border free provision of services, health professionals are required to be legally established in the Member State where they wish to pursue their profession. If Italy is the host Member State, the health professional gives prior notice to the Italian Ministry of Health by means of an annual declaration, that can be renewed beforehand if objective changes occur.If all the provisions under the relevant legislation are complied with, The Italian Ministry of Health, at the end of the assessment procedure, notifies the competent professional Association about the positive or negative decision concerning the professional’s application.
EU citizens with a health professional qualification, who wish to move to Italy for the first time to pursue their professional activities on a temporary and occasional basis, will previously apply to the Italian Ministry of Health, providing prior declaration concerning their intended transfer, together with all the necessary documents. Within one month from reception of this declaration, the Italian Ministry of Health gives notice of the results of the assessment process. If significant training gaps in the professional qualification of the applicant are highlighted in comparison with the professional training required in Italy, under Italian laws, compensatory measures will be applied.
The professionals who move to Italy for the first time from a third country and who wish to provide their services are required to inform the Ministry of Health giving 30 days’ prior notice, except for any urgencies, and they should submit the following documents:
- the Online Form C2, automatically generated by the system in PDF format, which will be either signed digitally with a qualified electronic signature as provided by eIDAS Regulation (EU) No. 910/2014, or printed, signed with a handwritten signature and scanned;
- written declaration specifying which service/s the professional intends to provide and which insurance policy covers his or her professional liability. This declaration is valid for the ongoing year and must be renewed if the provider wants to deliver temporary or occasional services in Italy afterwards;
- documents listed in Annex C2.
All the documents must be sent to the certified email (PEC), following the guidelines provided in the section ‘How can I apply’.

Remember to attach: application, payment RECEIPT of the stamp duty by bank transfer (NO order of payment and NO submission of bank transfer), PDF FILE of the whole necessary documentation (NO SCREENSHOT, NO PHOTO, etc). Each document will be submitted as a single corresponding pdf file (NO one overall pdf file grouping all the documents together). PLEASE NOTE: bank transfer is the only accepted payment method for the tax duty (NO STAMP DUTY).
